Pear Suite has closed a $7.6 million Series A round to expand its AI-powered platform supporting community health workers across the United States. The funding will help the digital health company grow its provider network and secure new contracts with Medicaid and Medicare health plans.

Rock Health Capital and Nexxus Holdings co-led the investment, joined by Mucker Capital, Enable Ventures, The SCAN Foundation, Acumen America, Impact Engine, and the California Health Care Foundation. The diverse investor group reflects strong confidence in Pear Suite’s approach to addressing gaps in population health through community-based care.

Why Community Health Workers Matter Now

Policy debates around Medicaid redetermination, work requirements, and preventative care reimbursement are shifting how population health services get funded and delivered. Community health workers have become an essential workforce because they build trust within their communities, understand cultural contexts, and help people navigate complex healthcare systems. Health plans now depend on CHWs to reach Medicaid members, older adults, people with disabilities, and others who have faced barriers to care.

“CHWs utilize their lived experience to provide culturally relevant health education, navigation, and advocacy that addresses the root causes of poor health outcomes,” said Colby Takeda, co-founder and CEO of Pear Suite. “Pear Suite isn’t just a technology company—we’re leveraging our platform to build a national provider network and deliver the contracting, clinical, and claims infrastructure that community-based providers need to thrive. With this financing, we plan to 10x the size of our provider network, continue expanding our reach into the Medicare market, and cement CHWs as a foundational part of the healthcare system.”

Growth Plans and Market Expansion

The company plans to multiply its provider network tenfold while pushing deeper into the Medicare market. Pear Suite aims to establish CHWs as a core component of the healthcare system rather than a supplemental service.

The new capital will accelerate platform development and support partnerships with additional health plans. By connecting community-based providers directly with payers, Pear Suite creates infrastructure that makes reimbursement practical and care coordination more efficient.

From Pandemic Response to National Platform

Pear Suite launched in 2021 during the COVID-19 pandemic, initially helping families access health and social services through culturally sensitive, technology-enabled support. The company has since grown to work with more than 300 provider organizations across the country. Over 2,500 CHWs, promotores, doulas, and other frontline workers now use the platform to close care gaps in their communities.

The company has partnered with more than 25 health plans to redesign how community-based care gets delivered and paid for. These partnerships align the interests of payers and providers while advancing health equity goals. Pear Suite’s platform handles the administrative and clinical workflows that community organizations need to participate in value-based care models.

Investor Perspectives on Market Opportunity

“The potential impact of scaling community health services is massive,” said Sean Day, Venture Principal at Rock Health Capital. “With a large workforce and reimbursement channels already in place, Pear Suite is the necessary puzzle piece to unlock sustainable funding models for community-based organizations nationally. And the need for Pear Suite’s infrastructure is only becoming more urgent amidst ongoing policy changes. Colby and Nick have done a fantastic job building a flywheel among payers and community health workers that’s creating meaningful impact for patients across the country. We’re thrilled to partner with them.”

“Community health workers play a vital role in promoting health and well-being, especially in communities historically underserved by our healthcare system,” says Veenu Aulakh, Director of Acumen America. “We are investing in Pear Suite because their platform gives CHWs the infrastructure to deliver sustainable, culturally-rooted care at scale—creating systemic change and a more equitable healthcare system for all.”
