A new report from Deloitte warns that companies are rolling out AI agents faster than their safety protocols can keep up. This rapid deployment raises serious concerns about security, data privacy, and accountability. Many businesses are moving from testing phases directly into full production without implementing enough safeguards.

Rapid Adoption Challenges Traditional Risk Controls

According to Deloitte’s survey, AI systems are transitioning from pilot projects to live environments at a pace that traditional risk controls struggle to handle. These controls were originally designed for human-centered processes, not autonomous AI agents. As a result, many organizations lack the necessary oversight to manage these evolving systems effectively.

Despite the fast growth, only 21% of companies have put strict governance measures in place for their AI agents. Meanwhile, 23% of organizations are already using AI agents, and this number is expected to jump to 74% within the next two years. Conversely, the percentage of companies not planning to adopt AI agents is projected to drop from 25% to just 5% during the same period.

Risks stem from Poor Governance, Not AI Itself

Deloitte emphasizes that AI agents are not inherently dangerous. Instead, the main risks come from weak governance and lack of context. When agents operate as autonomous entities, their decisions can become opaque and hard to understand. Without strong oversight, managing these decisions and preventing errors becomes very challenging.

Ali Sarrafi, CEO of Kovant, advocates for what he calls governed autonomy. He explains that well-designed agents with clear boundaries and policies—managed like any other employee—can work efficiently within safe limits. These agents should escalate issues to humans when decisions cross predefined risk thresholds.

He also highlights the importance of maintaining detailed logs of all actions and ensuring human oversight for high-impact decisions. This approach makes AI systems more transparent and trustworthy, transforming them from mysterious “bots” into manageable tools that can be inspected and audited.

Why Robust Guardrails Are Essential for Real-World Use

While AI agents may perform well in controlled environments, real-world business settings pose more challenges. Systems can be fragmented, and data may be inconsistent or noisy. This unpredictability can lead to unexpected behaviors, especially if agents are given too broad a scope or too much context at once.

Sarrafi notes that limiting the scope and decision-making capacity of AI agents makes their behavior more predictable. Breaking down complex tasks into smaller, focused actions helps prevent errors and makes it easier to intervene when needed. This structured approach also improves traceability, enabling early detection of issues and preventing mistakes from cascading.

The Importance of Accountability and Traceability

As AI agents begin to take concrete actions within business systems—such as logging their activities—managing risk and compliance becomes more straightforward. Keeping detailed records of every action allows organizations to trace back decisions, audit AI behavior, and ensure accountability.

This increased visibility offers a way to manage AI risks more effectively. It also helps companies meet regulatory requirements and build trust with customers. The key is to treat AI systems as part of the broader governance framework, with clear policies and oversight in place.
